Attached the new version of the team information sheet in Word. The traditional PDF versions are also great and accepted at tournaments. 

 

Judges uses the information sheets to help them remember teams. A team picture on the information sheet is a highly recommended.

 

You need to have 4 copies of the new or old version for tournaments. One for registration and 3 for the different judging rooms.
