We encountered some issues as well. But were able to insert an image into the document. A dialogue opened asking for an image when we clicked on the team picture field. It worked when the document was opened in Acrobat Reader or in IE. But it didn't work on Chrome. We didn't try Safari. Here is the source of the document that we used. Hope this helps.
