Thank you for your interest in 4-H !!
The attached forms must be completed by the youth and adults to become engaged in the 4-H program. The youth must complete the youth enrollment form, youth health history form, and a code of conduct. The adult must complete the adult volunteer enrolment form. Please return those to me at the address below.
As I indicated by phone, I would like to meet with you prior to your involvement in the April 19th event. I am available March 26th, March 31st, April 2nd or 3rd. Please let me know what is best for you.
