I can give you my personal perspective.  Only use gears, if you need gears.  If you need to gear up or down then it's worth it.  If you don't need to gear up or down, they become one more moving part that can go wrong and/or cause inconsistency.  I've always liked the idea of simplicity, the less things that can go wrong, the better.
