Any combination of printed FIRST Consent and Release forms and/or a roster from TIMS (Team Information Management System) that lists members and has the "Consent Form Status" of "Accepted" next to their names is fine.  At the team registration table also submit a Team Introduction Sheet.

Lloyd is correct.  If you have everyone showing an electronic consent form on the roster, then that's all you need.  You may also supplement that with paper forms for team members who do have the electronic version.
