I had the same problem, text boxes OK, couldn't add picture.I have a free pdf app called Foxit that allows you to edit/ad and save fields in a PDF. I added the picture with this.

If you are have issue inserting the picture, use a stand-alone Adobe Acrobat, not a web browser.  When you click on the image area, a dialogue will open.
