I am performing a study on how people make and evaluate important decisions in their lives.  Would you be willing to take 15 minutes to respond to a survey?  

In the survey, we ask you to describe your most important decision, and to answer some questions about it.  

There are no risks to being in the study.  You are free, of course, to terminate any time, free of penalty. Your responses will be anonymous and thus confidential.
