The ev3 software has a print option and it lets you print the entire program in a page or what you see in the page as well. You can force it to Landscape or Portrait...

Powerpoint or Keynote seem to be among the best options I've seen.

If you have really long programs try to grab 4-5 blocks at a go (and crop them fairly close vertically) so you can actually see.  If you have documentation in your project files you'll have to account for that too.

Yes, the only way to get a printout of your program is to take a screenshot of it and then paste it into a program like PowerPoint or MS Word. And then print THAT out.
